Our community is in need of a full -time Admissions Coordinator Candidate needs to be able to build a rapport with hospital social workers and other referral sources.
Must manage all incoming referrals including facility tours and be able to maintain a tracking system.
Screen all potential admissions for level of care needs and financial arrangements.
Initiate and complete the admission process including meeting with families, POA's and Guardians, completion of admission background checks, screenings, contracts, and ensuring a pleasant admission process for the new resident.
Must be familiar with Medicare, Medicaid, Managed Care, and secondary insurance plans processes.
Must be able to maintain a positive working relationship with Department Managers.
Represent the facility as the Community Liaison in a positive, professional manner to the community, families, and residents.
Experience with Point Click Care, Allscripts, Microsoft Word, and Excel.
